9. Complete the contact information for your group leaders, ensuring all email addresses provided are valid and can receive communications.
10. If applicable, select ‘Yes’ or ‘No’ to indicate whether tracking reports are required. This information helps in monitoring the progress of your group.
11. List participants' names and their corresponding roles (faculty, resident, or allied health professional), including their AAFP ID and module choices from the available drop-down list.
12. Verify that all necessary information is filled out accurately and completely before submitting the form.
13. Finally, save all changes made to the form, download a copy for your records, and email it to METRIC@aafp.org or fax it to (913) 906-6102.
