- In the Drainfield section, provide observations related to ponding depth, evidence of surfacing sewage, and maintenance status of both primary and reserve areas.
- Continue with the Pressure Distribution section, indicating the condition of clean-outs and orifices, if applicable.
- If relevant, fill out information regarding the Sand Filter, Mound, Proprietary/Aerobic Unit, and Disinfection Units, specifying whether the components are operational.
- Finally, address the System Status by noting if the system is failing, if deficiencies are identified, and any additional comments or observations.
