Specify the percentage of benefit you want to assign to this beneficiary, ensuring that the total for primary beneficiaries equals 100%.
- If you have a contingent annuitant, indicate this by checking the corresponding box in section 2, and ensure they are also listed in the primary beneficiary block. Sign and date the first page of the form before continuing.
- On the second page, enter your Social Security Number, full legal name, and date. Complete Section 3 to list additional primary and secondary beneficiaries. For each person, provide their Social Security Number or Tax ID Number, birth date, gender, legal relationship, telephone number, mailing address, and percentage of benefit. Again, the total percentage for secondary beneficiaries must also equal 100%.
- Finally, ensure that you sign and date the second page of the form before submission.
- Once you have completed the form, you can save your changes, download a copy, print it, or share it as necessary to submit to the ASRS for processing.
