If you are claiming a deduction of more than $500,000 for an item (or group of similar items) donated to one or more donees, you must attach the qualified appraisal of the property to your Form 8283 unless an exception applies.

Exceptions. Art valued at $20,000 or more. Clothing and household items not in good used condition. Easements on buildings in historic districts.

For noncash donations over $5,000, the donor must attach Form 8283 to the tax return to support the charitable deduction. The donee must sign Part IV of Section B, Form 8283 unless publicly traded securities are donated.
