UIA 1025, Employer Request for Address/Name Change, On-Line and Downloadable. The option of changing your business and/or mailing address is available to employers who have a Michigan Web Account (MiWAM).

The penalty is 10% of the contributions due on the un-filed report. (The penalty is applied once the report is filed and the amount of contributions due is determined.

Weekly Benefit Amount The amount you receive in unemployment benefits is calculated by multiplying the highest amount of wages paid to you in any base period quarter by 4.1 percent. The maximum benefit payment you can receive is $362.

A claimant who is discharged and disqualified for simple “misconduct” must requalify for benefits by “rework” before again becoming entitled to benefits. The rework requirement for “discharge” is 17 times the claimant's weekly unemployment benefit amount.

You must actively search for work and submit a record of your work search activities to be eligible for benefits. You are required to conduct at least one work search activity in each week that you certify for benefits. You must submit your work search activities at the time you certify.
