Follow the steps to complete your mentor application smoothly.

1. Click ‘Get Form’ button to access the mentor application form and open it in your preferred editor.
2. Begin filling out your personal information. Ensure to enter your last, first, and middle name as it appears on official documents.
3. Provide your student ID and current enrollment status. Select whether you are a full-time student (12 or more credits) or part-time (6 to 11 credits).
4. Input your major or program, followed by your WCTC email, preferred phone number, and personal email.
5. Fill in your home address, ensuring to include the street, apartment number (if applicable), city, state, and zip code.
6. Indicate your gender by selecting one of the specified options: female, male, or other. Then, provide your age and your race/ethnicity.
7. Select your shirt size for program events, then review and sign the Statement of Commitment section. Ensure you understand and agree to the commitments outlined therein.
8. Answer the provided questions to help the coordinators understand your communication preferences and expectations in the mentoring program.
9. Once you have completed the form, review all entries for accuracy before saving your changes.
