- Review the instructions carefully, especially the requirement stating that only active instate residents who have participated in the 2-day exam should fill out this form.
- Choose one of the designation options provided by checking only one box to signify your current status: Judicial Law Clerk, Employed in Law Firm or Legal Organization, Sole Practitioner, or Other.
- If you select 'Employed in Law Firm or Legal Organization,' print or type your mentor's name and Georgia Bar number. If your employer has not designated a mentor, check the appropriate box.
- If you identify as a 'Sole Practitioner,' you can nominate your mentor. Again, print or type their name and Georgia Bar number. If you need assistance finding a mentor, check the box and indicate whether you know your intended practice areas.
- For the 'Other' designation, confirm your compliance with TILPP if you are unemployed or not working with a law firm. A Group Mentoring environment will be arranged.
- In the certification section, print your name, provide your signature, and enter the name of the law school you attended. An original signature is essential; electronic submission is not permitted.
- After ensuring all fields are completed accurately, save your changes, and prepare to download, print, or share the form as needed.
