Don't forget to provide the date of your last arrival into the Philippines and your flight number.
6. In the School's Information segment, provide the registered address of your guardian and the school, including contact details. Ensure you accurately fill in the name of the school and its corresponding address.
7. Fill out the ACR I-Card section with your ACR number, contact number, issue date, and the course or degree you are pursuing. Also, include the valid until date and school year.
8. Review the Guardian section, which should not be filled out by you. This section will be completed by the Bureau of Immigration.
9. Ensure the certification section is completed by you, confirming that all information is true and correct. It will require your signature as the petitioner and the applicant, alongside the date.
10. Finally, check that all sections are filled accurately, save your changes, and download or print the completed form as needed. Follow the instructions regarding submission and further documentation.
