2. Begin with the student information section. Enter your name, including your last name, first name, and middle initial. Also, fill out the parent/guardian's name and their phone numbers, along with your school, grade, and student ID number.
3. Next, provide your email address and the name of your first period teacher. Make sure this information is accurate to facilitate communication.
4. In the Student Reflection section, write a thoughtful paragraph responding to the prompts regarding your service-learning activity. Reflect on the need your service addressed, who benefited from it, personal insights you gained, and connections to your classroom learning.
5. After completing your section, the Nonprofit Tax-Exempt Organization Information section is to be filled out by your supervisor. Ensure they include the organization name, address, tax-exempt number, and contact information.
6. Your supervisor should also describe the activity and record the dates of your service along with the total hours completed. They must provide their printed name, title, signature, and date.
7. If applicable, check the box for automatic hours attached to this activity, as instructed. Finally, submit the verification form to the SSL coordinator.
8. Once everything is filled out, review the information for accuracy before saving your changes. Download a copy for your records or print the form for submission.
