- In the 'From' section, provide the details of the releasing participant, which includes the designated REALTOR® of the firm, the office code, and the office name. Ensure that all details are accurate.
- Enter the listing agent's license number and name in the appropriate fields. Confirm that the agent will continue their membership with the firm.
- Indicate whether the agent is transferring their membership to the receiving firm, and remember that the transfer will not take effect until a Status Report is signed by the receiving participant.
- In the listing details section, confirm that all listings with an active status for the subscriber will be transferred. If there are specific listings to be transferred or those of another status, provide the MLS numbers below or attach a separate document.
- Date the form appropriately at the bottom where indicated.
- Obtain and include the signature of the releasing participant or the authorized person at the bottom of the form.
- In the 'To' section, complete the details for the receiving participant, including their designated REALTOR® of the firm, new office code, and office name.
- Fill out the new agent's license number and name.
- Date and obtain the signature of the receiving participant or authorized person.
- Finally, submit the completed form by faxing it to 214-637-2060 or emailing it to mls@dfwrealtors.com.
