- If declaring your annual salary, complete Part VI with your fixed annual salary earned, ensuring it is submitted by January 31 of each year.
- Finally, each section must include a declaration by the relevant representatives in Parts III, IV, and V, confirming that all information provided is accurate. Don't forget to sign and date the form as a Personalised Employment Pass holder in Part VII.
