  4. Input the complete street address, city, and zip code for the corporation’s principal office in California in Item 3. Do not include a P.O. Box or abbreviate the city name.
  5. If the mailing address differs from the principal office address, enter it in Item 4. Otherwise, leave it blank.
  6. For Items 5 to 7, list the names and complete addresses of the corporation's chief executive officer, secretary, and chief financial officer.
  7. In Item 8, specify the name of the agent for service of process in California, ensuring they have agreed to serve in this role.
  8. If an individual serves as the agent, complete Item 9 with their business or residential address in California. If another corporation is designated as the agent, leave Item 9 blank.
  9. In Item 10, check the box only if the corporation manages a common interest development; proceed to Item 11 if not.
  10. Complete Item 11 by typing or printing the name and title of the individual filling out the form and the date it was completed.

Every domestic nonprofit, credit union and consumer cooperative corporation must file a Statement of Information with the California Secretary of State, within 90 days after the filing of the initial Articles of Incorporation, and biennially* thereafter during the applicable filing period.

Completed forms along with the applicable fees can be mailed to Secretary of State, Statement of Information Unit, P.O. Box 944230, Sacramento, CA 94244-2300 or delivered in person (drop off) to the Sacramento office, 1500 11th Street, Sacramento, CA 95814.

All Statements of Information for limited liability companies can be filed online at bizfile.sos.ca.gov.
