- In Section A, fill out the required employment details, including the Date of Joining and Date of Retirement. Ensure these fields are filled in correctly as they are mandatory.
- Input your Employee ID, Corporate Registration Number, and CBO Number allotted by CRA.

- Proceed to Section B to make changes in your scheme preference. Select your Preference Fund Manager (PFM) by ticking one box from the list provided.
- Choose your Investment Option: either Active Choice or Auto Choice. If selecting Active Choice, proceed to fill out the Asset Allocation details in the table provided.
- Ensure that your total asset allocation across Equity, Corporate Bonds, and Government securities equals 100%. If not, your application may be rejected.
- Complete the declaration and authorization section to affirm that the information provided is accurate and true.
- Affix your signature or thumb impression as required.
- Conclude by saving your changes. You may download, print, or share the completed form as needed.
