PS Form 3546, Official Change/Correction to Mail Forwarding Change of Address Order, is an official document whereby employees make corrections to the spelling of a name or elements of a mailing address.

The Change of Address Order is a hardcopy paper form that can be completed to submit a change of address. If you mail or submit a PS Form 3575 without going through proper identity verification at a post office, you will receive a letter in the mail.

Address Service Requested provides forwarding and address correction services when possible. If the mail is undeliverable and not forwardable, it is returned to the sender with the reason identified on the mailpiece. Change Service Requested provides address correction services without forwarding or return.

Address Service Requested provides two services: (1) the mail piece is forwarded to the new recipient address if possible and (2) the sender receives back a USPS®-sent postcard called a Form 3547 (Notice to Mailer of Correction in Address) which details either the new address or reason for non-delivery.
