- Complete the credit card information by indicating the type of card you are using, your account number, and the expiration date. Confirm whether you want to authorize automatic renewal of fees and initial the section as required.
- Sign and date the application to confirm your agreement with the terms and conditions of individual membership and exchange. Ensure that you keep a copy of the completed application for your records.
