- Provide your full name as listed on your water bill or valid identification.

- Fill in your home address, including city, state, and zip code.
- If different from your home address, provide your mailing address, including city, state, and zip code.
- List the daytime telephone number, including the area code.
- If you are receiving low-income assistance from another utility for your electric or gas bill, indicate the name of that utility.
- Make sure to submit a copy of your current picture ID, recent water bill, and proof of 30-day income (such as pay stubs or other income documentation) along with the application.
- Review the income eligibility criteria: a single-person household must not exceed $17,505 annually, while a two-person household must not exceed $23,595. Note the requirement to inform EPCOR Water if you no longer qualify for assistance.
- Sign the application where indicated and enter the date of submission.
- Mail the completed application to WACOG at the provided address.
