You must require the use of a private vehicle for transportation. You must have a severe, permanent disability that impairs mobility as certified by your personal physician and a New York City physician designated by the Department of Health and Mental Hygiene (DOHMH) Medical Certification Unit (MCU).

You may apply for a replacement Persons with Disability Parking Placard by completing Form MV-145A, “Persons with Disability Parking Placard Application.” Your previous placard number will be listed as the previous placard on PennDOT's computer system.

The State Parking Permit for Persons with Severe Disabilities (known as the "blue hang tag") is now valid for 5 years from the date it was issued. After 5 years, the permit expires and you must re-apply by submitting a new application to the Department of Transportation (DOT) by mail.

Most accept Form MV-664.1, which is available at any DMV, DMV call center, or online. However, some localities have their own application form, and not all localities issue temporary permits.
