- Combine the amounts from lines 15 and 16 and enter this total in line 17.

- If this report is filed late, include a $10 late report penalty in line 20.
- Add any prior balances or subtract any owed credits in line 21 to ensure your totals are accurate.
- Finally, in line 22, sum lines 17, 18, 19, 20, and 21 for your total amount due this quarter.
- Enter the amount you are remitting in line 23, which should match line 22.
