- In the Document (Supersedes/Supplements) section, indicate if the document overrides or complements an existing GIDEP document. If neither, write 'None'.
- For Environmental Exposure Codes, enter the appropriate single symbol coding defined in the Subject Thesaurus; use N/A if not applicable.
- Input the Manufacturer information, including the abbreviation and H-4 Code number from the GIDEP Manufacturer List. Refer to Block 15 for further details if necessary.
- Enter the complete Manufacturer Part Number, replacing letters and numerals as indicated in the guidelines. If multiple part numbers are applicable, include them in Block 15.
- For the Industry/Government Standard Number, submit relevant standard part numbers as required, indicating any supplementary standard numbers in Block 15.
- In the Outline, Table of Contents, Summary, or Equivalent Description, briefly summarize relevant details of the document that could assist users.
- Provide key words that enhance the information retrieval for the subject matter contained in the document, ensuring they are clear and free of abbreviations.
- Finally, include the name and signature of the GIDEP Representative, as well as the name, city, state, and GIDEP code of the submitting participant activity.
- Once all fields are accurately completed, save your changes. You may also choose to download, print, or share the completed form as needed.
