Get Appendix Form

An appendix contains information that supplements the reader's understanding of your research but is not essential to it. For example: Interview transcripts.

The heading should be "Appendix," followed by a letter or number [e.g., "Appendix A" or "Appendix 1"], centered and written in bold. Appendices must be listed in the table of contents [if used]. The page number(s) of the appendix/appendices will continue on with the numbering from the last page of the text.

What is an appendix? A section at the end of a paper that includes information that is too detailed for the text of the paper itself and would "burden the reader" or be "distracting," or "inappropriate" (APA, 2019, p. 41-42).

If your paper only has one appendix, label it "Appendix" (without quotes.) If there is more than one appendix, label them "Appendix A," "Appendix B," etc.

Appendix B to 12 CFR part 1003 provides a data collection model form for collecting information concerning an applicant's ethnicity, race, and sex that complies with the requirements of § 1002.13(a)(1)(i)(B) and (ii).
