- Input your mailing address, including your apartment number (if applicable), street number, street name, city, province or territory, and postal code. Indicate whether this is a new address.
- If you moved to a new address, select the date of your move by entering the year, month, and day.
- Select the options for payment types. Tick the box for income tax refund, GST/HST credit, WITB advance payments, and any other overpayment of tax to have these amounts deposited into your bank account held in Canada.
- Provide your banking information. Enter the branch number (5 digits), institution number (3 digits), and account number (maximum 12 digits). You may also attach a blank cheque marked 'VOID' for this account.
- If you receive Canada Child Tax Benefit payments, select whether you want it deposited into the same account or a different one. If different, provide the required banking details as mentioned in the previous step.
- For Universal Child Care Benefit payments, follow the same process as for the Canada Child Tax Benefit, selecting the appropriate option for your banking preferences.
- In the certification section, provide your signature, telephone number, and the date to authorize the Receiver General to deposit payments into your selected bank account(s).
- After completing the form, review all entered information for accuracy. Once confirmed, you can save changes, download, print, or share the completed form.
