Follow the steps to complete your Direct Deposit form.

1. Press the ‘Get Form’ button to access the Direct Deposit form and open it in the editor.
2. Begin filling out your identification details. Enter your first name and initial, followed by your last name. Next, provide your social insurance number.
3. Input your mailing address, including your apartment number (if applicable), street number, street name, city, province or territory, and postal code. Indicate whether this is a new address.

8. For Universal Child Care Benefit payments, follow the same process as for the Canada Child Tax Benefit, selecting the appropriate option for your banking preferences.
9. In the certification section, provide your signature, telephone number, and the date to authorize the Receiver General to deposit payments into your selected bank account(s).
10. After completing the form, review all entered information for accuracy. Once confirmed, you can save changes, download, print, or share the completed form.
