- If applicable, Section C is for police certification, which should be completed if the health care provider section is not filled out.
- For veterans, Section D requires certification from a Veterans Administration representative, stating any service-connected disabilities.
- Section E is the notarization and applicant signature section. Ensure the applicant, or authorized person, signs in the presence of a notary public.
- Review all filled-out sections for accuracy. Save your changes in the editor, and proceed to download or print the completed form.
- Submit the completed form to the PA Department of Transportation Bureau of Motor Vehicles as specified in the instructions.
