5. Ensure that academic representatives from your respective departments sign the form, confirming their approval for each course.
6. Check all the boxes in the 'Acknowledgment' section to confirm you have read the regulations, understand academic standing requirements, and acknowledge responsibilities regarding your transcript.
7. Sign and date the form in the 'Student’s Signature' field. Ensure that your academic advisor also signs and dates the form after discussing the implications of off-campus study.
8. Once all sections are complete and signatures are obtained, submit the form for review where indicated. Be advised that the request may take some time for processing.
9. After submitting the form, ensure to keep a copy for your records and follow up if you do not receive confirmation within the expected timeframe.
