9. Describe the qualities that best represent the applicant in the provided space.
10. Answer whether the applicant has a historically disadvantaged background and explain why, if applicable.
11. Outline your reasons for believing the applicant qualifies for the EOP and suggest any needed services or assistance for their college success.
12. Discuss any barriers to achievement the applicant may have faced and whether you believe they will impact their college performance.
13. Give your assessment of the student's potential and motivation for undertaking college work.
14. Finally, sign and date the recommendation, then return the completed form to the EOP Office at the intended campus.
