- Proceed to Section A and answer the three questions with a 'Yes' or 'No' response.
- In Section B, evaluate your experiences in Wraparound by indicating your level of agreement with each statement.
- Move on to Section C to assess the frequency of various activities during your wraparound team meetings.
- In Section D, respond with 'Yes' or 'No' to questions regarding your satisfaction and your child's progress in the wraparound process.
- Finally, in Section E, provide feedback on the survey itself, rating your satisfaction with the ease of completion and relevance of the questions.
- Once all sections are completed, ensure to save your changes, and you may also download, print, or share the completed form.
