- Check the 'Trial' box if you were scheduled for a trial which guarantees a minimum of four hours.
- Document the date of your assignment.
- Record the official scheduled start time of the hearing, ensuring it matches the time noted by the court, and not when you checked in.
- Document the end time of your assignment accurately.
- Provide the name of the Judge or Master assigned to the case.
- Enter the Courtroom number and location.
- Input the number of hours to be compensated, rounding to the nearest quarter hour.
- Select the correct hourly rate for your interpreting services based on your certification status.
- Calculate the total amount due by multiplying the hours paid by the hourly rate.
- If mileage is applicable, document the quantity of miles to be compensated.
- Indicate the reimbursement rate per mile based on current rates.
- Calculate the total amount for mileage by multiplying the quantity of miles by the reimbursement rate.
- If you incurred parking expenses, include that amount, and attach receipts when possible.
- Calculate the round trip travel time, rounded to the next quarter hour, confirmed by using MapQuest.
- Identify the correct hourly travel rate, which is half of the regular interpreting rate.
- Multiply the travel time by the rate to find the total amount due for travel.
- Record the date and time when you were notified of any cancellation.
- Check the 'Upon Arrival' box if you were notified of cancellation while at the courthouse.
- Check the appropriate minimum box, and if applicable, enter the amount due to cancellation.
- List any additional charges that may apply, ensuring they are documented and receipts are attached.
- Sum all relevant amounts to determine the total reimbursement.
- Sign the form to attest that the entered information is correct. If working for an interpretation company, have a representative sign.
- An authorized Judiciary employee must print their name, sign, date, and provide their title to approve the invoice.
