- Navigate to Section 3, the Worksites list. Check the name, street address, and description for each worksite. Make corrections as needed to ensure they are accurate and uniquely identifiable.
- Enter the number of employees for each month of the quarter. Include all full-time and part-time employees who worked during or received pay for the pay period that includes the 12th of the month.
- Input the quarterly wages for each worksite. Wages should be rounded to the nearest dollar and include all amounts that fall under State Unemployment Insurance laws.
- Use the Comments section to explain any significant changes in employment or wages, such as store closures or seasonal variations.
- If any worksites are closed, sold, or inactive, document this in the Comments section indicating the closure date, the new owner's name (if applicable), and their U.I. Number if known.
- Check if the list of worksites is complete. If there are additional worksites that are not listed, provide the necessary information as stated in the instructions.
- Complete the Totals section at the end of the list, summing the employees and wages for all worksites. Ensure these totals match your Quarterly Contributions Report.
- After completing the form, save any changes, download, print, or share the form as needed, and prepare it for submission.
