Filling out the School Immunization Data Reports (Idr) online is essential for ensuring that your school's immunization data is submitted accurately and efficiently. This guide will walk you through each step of the process to complete the form confidently.

Follow the steps to complete the Idr online.

1. Click the ‘Get Form’ button to access the form and open it in the online editor.
2. Use the tab key or your mouse to navigate through the form fields; refrain from using the enter key for the best experience.
3. Select the appropriate grade level for your report, which can be kindergarten, 6th grade, or 10th grade.
4. Public and charter schools need to input the CTDS number for their institution. You can find the CTDS numbers on the Arizona Department of Education website.
5. For private schools, enter the school’s phone number in the field designated for the CTDS number.
6. Provide the mailing address for the school in the corresponding section of the form.
7. The form will automatically calculate the number of students who do not meet the vaccination requirements based on the total enrolled students and the number who meet the requirement.
8. If you cannot finish the form in one sitting, click the ‘Save’ button to save your progress. You can later retrieve it by using the ‘Search’ function with the CTDS number or school phone number.
9. Once you have filled in all required sections of the form, click on ‘Completed.’ The top part of the completed form will stay visible, allowing you to select another grade level if needed and complete the report for that level.
10. The form will evaluate compliance with vaccine requirements by referencing student ages, the interval between vaccine doses, and any exemptions.
11. To verify that your form has been submitted, use the 'Search' button and enter the CTDS number or phone number to locate your school.
