7. If applicable, calculate any additional contributions for added years or AVCs and enter them in boxes (e) and (f), respectively. Finally, sum the values in boxes (d), (e), and (f) and write the total in the Grand Total box (g).
8. Once all information is accurately filled in, ensure all required documents, such as the completed GP Locum A forms and a cheque for the grand total, are attached. Send the completed form to your host HA/PCT before the 7th day of the month following the reporting month.
