- In Part 1, record the name of the GP practice and its associated practice code. Complete this section accurately to ensure proper identification.
- Part 2 of the form is for HA/PCT use only. Enter your total NHS locum gross pay received for each GP practice you worked for in the previous calendar month. Make sure to also attach the corresponding GP Locum A forms for each payment declared.
- Calculate your NHS pension contributions in Part 3. Total your gross NHS locum pay and enter that figure in box (a). Calculate and input 10% of that amount in box (b). Deduct the value in box (b) from box (a) and enter the result in box (c).
- Determine the NHS Scheme gross employee contributions by calculating 6% of the amount found in box (c) and noting that figure in box (d).
- If applicable, calculate any additional contributions for added years or AVCs and enter them in boxes (e) and (f), respectively. Finally, sum the values in boxes (d), (e), and (f) and write the total in the Grand Total box (g).
- Once all information is accurately filled in, ensure all required documents, such as the completed GP Locum A forms and a cheque for the grand total, are attached. Send the completed form to your host HA/PCT before the 7th day of the month following the reporting month.
- Remember to save changes to your document, and consider downloading or printing a copy for your records before submitting the forms.
