7. Complete the 'Emergency Contacts' section for two contacts, providing their names, phone numbers, relationship to the child, and addresses.
8. In the 'Doctor/Health Fund Details', enter the doctor's name, phone number, address, and relevant Medicare or private health fund details.
9. Provide details of the collection contacts, again including names, addresses, and phone numbers in the respective fields.
10. In the 'Child Other Details' section, state if the child lives with anyone other than the parent, their place of birth, languages spoken, and the parent's activity status.
11. Indicate whether the child identifies as Aboriginal or Torres Strait Islander, and whether the child has a disability or special needs.
12. Fill in the 'Child Interests' section with the child's favorite sport, hobby, food, and any other relevant information.
13. Finally, review the 'Centrelink Claim' section to note the number of children for whom you are claiming CCB percentage.
14. Once all sections are completed, ensure you save your changes, and choose the options to download, print, or share the enrolment form as needed.
