- Fill out the Declaration section, confirming that all information provided in the form is true and complete to the best of your knowledge. Include the name and signature of the Life Assured or Policyowner and the date of signing.
- Complete the Nature of Claim section by providing detailed descriptions of the illness or injury, including the date of first consultation and previous medical history related to the claim.
- If applicable, fill out the Death Benefit section if you are filing for the Death of the Life Assured, providing necessary details about the deceased, including medical history and marital status.
- After completing all relevant sections, ensure all mandatory documents are attached. This includes the PruMum2Be Claim Form, Medical Specialist Report, Clinical Abstract Application Form, and any necessary medical reports.
- Once all sections are filled and documents attached, review and save your changes. You can then download, print, or share the completed form as needed.
