- In the enrollment information section, check the appropriate option indicating your current enrollment status with the course.
- Sign and date the form before submission. If applicable, remember to complete the GPA Adjustment Form for adjustments in GPA calculations.
- Submit the completed form with required signatures to Curriculum Advising, and retain a copy for your records. Allow five business days for processing.
