- Fill in the item number and ensure that the total does not exceed 20 in the sixth box.
- In the seventh box, specify the number of packages and provide a detailed description of the goods. Make sure it aligns with both the invoice description and the HS code.
- The eighth box requires the HS code for each item; this is a six-digit classification necessary for customs.
- Specify the origin criterion in the ninth box. This governs the goods' eligibility for preferential treatment under the trade agreement.
- Indicate the gross weight and quantity in the tenth box, using the appropriate measurement units.
- Enter the invoice number and date in the eleventh box. If invoiced by a non-Party operator, provide that operator’s invoice details.
- State the invoiced value in the twelfth box. Include the value of the original commercial invoice where necessary.
- In the thirteenth box, ensure the declaration by the exporter is completed, signed, and dated.
- Finally, the fourteenth box must be completed, signed, dated, and stamped by an authorized representative from the body certifying the document.
- After completing all sections, save any changes made, and then download, print, or share the completed Annex 5 Certificate as needed.
