- Fill in the Policy Account Number and Transit ABA Routing Number where indicated.
- Review the statement confirming responsibility for any changes in coverage, ensuring you understand your obligations.
- Complete the form by signing in the Participant Signature area and date it by writing the current date.
- Return the completed form to the specified address or fax number provided at the bottom of the form.
