The CPOT includes evaluation of four different behaviors (facial expressions, body movements, muscle tension, and compliance with the ventilator for mechanically ventilated patients or vocalization for nonintubated patients) rated on a scale of zero to two with a total score ranging from 0 to 8.

Directives of use of the CPOT 1. The patient must be observed at rest for one minute to obtain a baseline value of the CPOT. 2. Then, the patient should be observed during nociceptive procedures (e.g. turning, wound care) to detect any changes in the patient's behaviors to pain.

The CPOT was developed for the assessment of pain in critically ill patients. The scale consists of four behavioral domains: facial expression, body movements, muscle tension and compliance with the ventilation for intubated patients or vocalization for extubated patients.

The CPOT can be used to assess intubated or sedated patients pain based on facial expressions, muscle tension and movement as well as compliance with ventilated breaths for intubated patients or vocalized pain for non-intubated patients.

We considered a BPS score 3-4 and CPOT score 0-1-2 like absence pain; a BPS score 5-6-7 and CPOT score 3-4 like moderate pain; and a BPS score 8-9-10-11-12 and CPOT score 5-6-7-8 like severe pain.
