- In Section 1, select the appropriate 'Class of Certificate' and 'Certificate Type.' Specify the validity period by selecting either one year or two years.
- Complete your personal details, including 'First Name,' 'Middle Name,' and 'Last Name.' Provide your 'Date of Birth' using the DD-MM-YYYY format. Choose your 'Sex' as per preference.
- Fill in your ‘Contact Address Details’ using block letters. Include your complete address as per your bank records. Fields include 'Flat/Door/Block No.,' 'Name or Premises,' 'Road/Street,' 'Area,' 'Town/City,' 'State,' and 'PIN Code.'
- Provide a valid email address that will be used to receive your digital certificate. Ensure to use this same email when enrolling for the Digital Certificate online.
- In Section 3a, enter the details of your government-issued photo identity proof. Attach a photocopy of the ID and ensure it is attested by an authorized person.
- If applicable, specify your address proof in Section 3b. Ensure this is attested by a SafeScrypt Prevalidated Signatory.
- Sign the declaration to affirm that all information is true, date the application, and provide your place of application.
- Ensure the application is attested either by a bank manager or a SafeScrypt Prevalidated Signatory, who will also provide their signature and details.
- Final review: Ensure all mandatory fields (marked with '*') are completed and that the form is legible. Save your changes, then download or print the form to submit.
