- Indicate your participation in the Wellnys Everyday Olympic Experience by checking either the Individual or Family registration type, and specify the number of adults and children if applicable.
- Review your details for accuracy before submitting. Make sure to double-check your email address and mailing address, as these are critical for communications.
- After completing the registration form, submit it by either emailing it to wellnys@eap.ny.gov, faxing it to (518) 486-9796, or mailing it to NYS EAP, 55 Elk Street, Suite 301, Albany, NY 12210 by the registration deadline of July 13.
- Once you have participated and documented your daily healthy behaviors, fill out the Olympic Experience Tracking Form with the completed behaviors and their corresponding dates between July 27 and August 12.
- Submit your tracking form by either emailing it to wellnys@eap.ny.gov, faxing it to (518) 486-9796, or mailing it to NYS EAP, 55 Elk Street, Suite 301-A, Albany, NY 12210 by the final deadline of August 24.
- To ensure everyone in your family receives recognition, each family member must complete their own tracking form. Parents should submit one form for each child.
