- Complete contact details for overseas schemes, if applicable, including the scheme auditor details.
- Answer whether you have ever been issued with an Employer’s Contracted-out Number (ECON) and whether you’ve been included on another employer’s contracting-out certificate.
- Identify the method of revaluation for Guaranteed Minimum Pension (GMP) rights and specify details regarding employer information, including Company Registration Number where appropriate.
- Declare the information provided is correct. The signature must come from the person responsible for managing the scheme or the employer. Ensure the declaration is dated appropriately.
