5. Document the detailed scope of work, including a condition report for existing vaults. Make sure to describe any damage or deterioration and include historical data such as the year built.
6. Prepare a detailed sidewalk plan that illustrates work limits, the property line, curb line, and all street furniture within 25 feet of the job limit. Clearly identify the entire extent of the vaults.
7. Construct a detailed full cross-section showing overall dimensions, including curb and vault wall specifications. Indicate any elements that are existing, to remain, be repaired, or replaced.
8. Draw the sidewalk profile representing the elevations at specified distances past the project limit lines, particularly for projects involving new grades.
9. Compile a photo log with descriptive notes on location, including inside and outside conditions, complemented by a photo index.
10. Provide detailed drawings for hatches, hold doors, and any sidewalk openings, along with manufacturer cut sheets.
11. Include a pedestrian level of service study if applicable, along with a location feasibility study where necessary.
12. For transformer vaults, ensure to add the Con Edison layout.
13. Finally, complete the BPP, first floor, and cellar floor plans, or structural plans for existing or new building vaults, ensuring approval from the Department of Buildings.
14. Once you have filled out all required fields and sections thoroughly, save your changes, and you may choose to download, print, or share the completed form as needed.
