Follow the steps to complete your immunization record form.
- Click ‘Get Form’ button to obtain the Immunization Record Form and open it in your preferred editor.
- Complete your personal information in the designated fields. This includes your name, address, contact number, email, and date of birth.
- Indicate your first term of enrollment by circling the appropriate option: Fall, Spring, Maymester, or Summer sessions.
- Select your status as a student, such as Freshman, Transfer, International student, or Distance education, by marking the relevant checkbox.
- In Section A, fill out the required immunization information with dates of administration for MMR and meningococcal vaccines. Ensure to sign where indicated.
- Answer the tuberculosis screening questions honestly. If any risk factors apply, refer to page 3 and complete the Tuberculosis Risk Assessment.
- If applicable, complete Section B for optional immunizations, including HPV and Tetanus, by noting the dates for the doses received.
- If you are under 16, ensure that a parental consent form in Section C is completed and signed.
- If you have any exemptions, fill out Section D, providing the necessary documentation as described.
- After reviewing all entries for accuracy, save your changes, download the form, and print or share it as required. Return the completed form by mail or fax to the specified address.
