- If applicable, include your maiden or former names while attending Malone in the provided field.
- Fill out your current address, city, state, and zip code.
- Enter your home phone and cell phone numbers in the appropriate fields.

- Complete the payment section, choosing to pay by cash, check, money order, or credit/debit card. Fill in the necessary card information.
