- Record the length of time the customer has resided at their current address.
- Answer whether the customer has resided at any other addresses in the time frame provided and, if so, list those addresses with corresponding timeframes.
- Select all options that apply to the customer's explanation for the address discrepancy.
- Provide transaction status information by indicating whether the transaction was consummated and, if not, the reason why.
- Indicate whether the dealership has formed a reasonable belief about the customer's identity and address based on verification policies.
- Finally, fill in the individual submitting the form's name, title, and signature.
