Follow the steps to fill out the Form 4a online.

- In Section I, provide your personal information. Fill in your social security number, birth date, and name as it appears on your Application for Licensure (Form 1). Make sure to include your mailing address, and daytime phone and email address.
- Also in Section I, enter the name of the firm or organization where your experience was gained, and fill in the name and address of your endorser. Indicate the beginning and ending dates of your employment, along with whether you were a full-time or part-time employee.
- In Section II, describe your professional experience. Begin with a general overview of your surveying duties and your level of responsibility while employed at the firm. Include any relevant changes in your job title and attach additional sheets if necessary.
- Continue in Section II by detailing specific work you performed during your employment. Indicate the time spent on each project without exceeding the actual calendar time made during your employment.
- Make sure to sign and date the Attestation at the end of Section II before sending the complete form to your endorser. Ensure that you use a separate Form 4a for each endorser.
- Section III should be completed by your endorser. They will need to read your description of experience, provide their verification, and sign the attestation. This signature must be notarized.
- Finally, instruct your endorser to return the completed Form 4a directly to the Office of the Professions at the specified address, ensuring it is not returned by you.
