- In Section 3, Authorized Approval, ensure you enter the name, signature, date, telephone number (including country code if necessary), and email address of the authorized individual responsible for submitting the form.
- Once completed, review all entries for accuracy. You can then save any changes, download, print, or share the form as appropriate.
