- For non-spousal beneficiaries in Part II, similarly select a lump sum payment or a direct rollover. If opting for a direct rollover, provide the corresponding trustee information as required.
- In Part III, sign and date the application to acknowledge your intent to apply for survivor benefits. Ensure that this section is completed and notarized if required.
- In Part IV, get the employer certification completed by the appropriate agency. This involves indicating whether the deceased member had terminated employment prior to their death.
- Review the completed application to ensure all information is accurate. Attach the certified original death certificate as required. After checking for completeness, save changes and proceed to download, print, or share the form as needed.
