A Form SSA 11-BK is known as a Request to be Selected as Payee. It will be used by someone who wants to become a representative payee for another payee. This form must be completed in a face-to-face interview with someone from the Social Security Administration in order to determine eligibility and suitability.

It is called Representative Payee Report of Benefits and Dedicated Account, SSA-6233-BK. You should keep these records (e.g. bank statements and canceled checks) along with receipts for two years from the time you complete the form.

You must complete form SSA-11 (Request to be selected as payee) and show us documents to prove your identity. You will need to provide your social security number, or if you represent an organization, the organization's employer identification number.

The SSA-552 describes how to use and how not to use dedicated account funds, and the reporting and accounting responsibilities relevant to dedicated accounts. By signing the form, the representative payee acknowledges that he or she understands the usage restrictions of the dedicated account.
