- In section C, list all federal awards and expenditures. Include the federal agency, any pass-through grantor, program name with CFDA number, grant number, grant revenues, and grant expenditures. If necessary, attach additional pages for complete disclosure.
- In section D, ensure that the form is signed by an eligible individual, such as the executive director or chief financial officer. Fill in the printed name, title, signature, and date of signing.
- Finally, submit the completed GACFED form within 60 days after the end of your fiscal year to the specified WV DHHR address. You may also wish to save changes, download, or print the form for your records.
