Parainfluenza virus most commonly causes viral croup or acute laryngotracheitis, primarily types 1 and 2. Other causes include influenza A and B, measles, adenovirus, and respiratory syncytial virus (RSV).

Croup is a common, primarily pediatric viral respiratory tract illness. As its alternative names, acute laryngotracheitis and acute laryngotracheobronchitis, indicate, croup generally affects the larynx and trachea, although this illness may also extend to the bronchi.

The parainfluenza virus is the most common cause of croup. Four strains of the virus are responsible for most croup cases.

The condition most commonly confused with croup is possibly laryngomalacia, which also presents with stridorous breathing, either when the child has a viral infection or is simply upset.

Diagnosis of croup is based on clinical findings of barking cough, stridor, and hoarseness. Diagnostic testing is typically not necessary. Humidified air inhalation does not improve symptoms in patients with moderate croup. Corticosteroids should be administered to patients with croup of any severity.
