- Input the declared value as stated on the shipment, along with the merchandise value, FedEx pack & ship fee (if applicable), and freight charge.
- Summarize the total claim or C.O.D. amount accurately to ensure proper reimbursement.
- In the customer remarks section, provide any additional information relevant to the claim.
- If salvage is necessary, explain why and provide contact information for the salvage pickup service.
- Enter claimant’s information, including their contact details and signature. Ensure you date the form properly.
