Next, input your mailing address, including apartment number (if any), city, state, and zip code. Additionally, include your day and alternate phone numbers, and your email address.
- In 'Order Clipper Card' section, select the type of card you wish to order by checking either the 'Youth' or 'Senior' option.
- Complete the 'Certification of Eligibility' section by entering your date of birth in MM/DD/YYYY format. Then, check the type of document you are using to verify your eligibility (e.g., Birth Certificate, Passport, etc.). Write the identification number of your eligibility document and the agency that issued it.
- In the 'Signature' section, affirm the accuracy of the information by signing your name and entering the date. If the applicant is a minor, a parent or guardian should sign on their behalf.
- If you wish to create an online account, answer 'Yes' or 'No' in the 'Create Online Account (Optional)' section. If you choose 'Yes', provide a temporary password with a minimum of 8 characters. Remember to change this to a permanent password after receiving your Clipper Card.
- Once all sections are filled out appropriately, save your changes. Depending on your needs, you may choose to download, print, or share the completed application.
